Asylum Storyline of Moon Knight
The storyline of the asylum in Moon Knight is one of the most confusing and pivotal parts of the series. Here is a breakdown of what's happening, explained in points:
The "Asylum" Is Not a Real Asylum
It's the Duat: The "asylum" is a manifestation of the Egyptian afterlife, specifically the Duat, which is the underworld.
Because Marc Spector has dissociative identity disorder (DID), his psyche projects the environment as a psychiatric hospital, with Arthur Harrow as the head psychiatrist.
A Purgatorial Journey: Marc and Steven's "stay" in this asylum is a journey through the Duat after they were killed by Arthur Harrow.
Their "therapist," the hallucinated Dr. Harrow, is trying to get them to confront their past and balance their scales.
The staff and other patients are also people they've encountered in their life, adding to the psychological nature of the setting.
What Is Actually Happening: The Afterlife Explained
They Are Dead: Marc and Steven were shot and killed by Arthur Harrow. Their consciousness is now on a boat sailing through the Duat.
Taweret, the Guide: The hippo headed goddess, Taweret, is the real guide of their soul. She appears as a friendly, maternal figure. Her job is to guide souls through the Duat to the next stage of the afterlife.

The third guy in Moon Knight
You've hit on the biggest mystery and most important reveal in the entire Moon Knight series: the third personality.
That "third guy" is Jake Lockley.
Here's what's going on with him:
He's the Most Brutal Alter: In the comics and the show, Jake Lockley is a cab driver and a much more violent, ruthless personality than either Marc Spector or Steven Grant. He's the alter that is willing to do things the others won't, including killing without hesitation.This is why Khonshu has an arrangement with him.
The Blackouts: Throughout the series, there are several moments where both Marc and Steven "black out" and wake up to a scene of carnage, with no memory of what happened.
They are often surrounded by dead or brutally injured people. This is Jake taking over. For example, in the third episode when Marc is interrogating the thugs, he blacks out and wakes up on a cliffside with the thugs dead and a bloody knife in his hand. Neither Marc nor Steven knows how it happened—it was Jake.
He Was There All Along: The show subtly hints at his presence from the very beginning. Remember the third sarcophagus in the asylum? It was rattling and shaking. That was Jake, locked away from Marc and Steven's consciousness. When they finally accept each other and are in the afterlife, Jake's part of their mind is still contained, so he isn't part of their journey to balance the scales.
The Final Act: At the end of the series, after Marc and Steven defeat Harrow, they refuse to kill him, despite Khonshu's urging. They tell Khonshu they are done with him and demand to be released from his service, which he reluctantly does. However, in the post-credits scene, we see a seemingly defeated and drugged-up Harrow being wheeled out of a psychiatric hospital.
The man pushing the wheelchair is Jake Lockley, who speaks fluent Spanish. He puts Harrow in a white limousine with Khonshu, who reveals that while he may be done with Marc and Steven, he's not done with their body because he still has a deal with Jake.
Khonshu says, "Why would I ever need anybody else when he has no idea how troubled he truly is?"
Jake then shoots and kills Harrow, effectively finishing the job and proving his ruthless nature.
